I added a number 3 to the OP. This was also part of the whole inspiration for this thread but had totally escaped my mind by the time I wrote it.
I completely agree with you on boar knots. I find if the hairs in the brush aren't fully hydrated to start with, that they rob the water from your lather as the brush sits there between passes.
Personally, between my first pass and my second, I usually add more product anyway. Be it soap or cream it doesn't matter, 98% of the time I'm going back to the tub for a few more swirls. Then again, I don't try to load my brush for a full shave to start because I know I'll be adding more product before I need it again. It doesn't feel wasteful and most product is cheap enough on a per use basis that I don't even notice. Plus, probably like most people here, I acquire soap and cream at a faster rate than I use it.

As to the lather, for me it depends on the fibers of the knot. Boar seems to be best on the first lather, but for others, maybe 2nd pass is better? I haven't really paid attention!
