I think it is pretty simple to create your own template with just about any spreadsheet.  I have one I use in Excel.  I headed the columns as follows: 
Date;  Razor;  Blade;  No. of Shaves (on the blade);  Soap or Cream;  Brush and the last column is for Shave Comments. 

Some comments about the spread sheet.  First, you can add or delete columns to fit your needs,  second, the column width can be adjusted to fit the data (narrower for date, and No. of Shaves and wider for the other columns, and third,  Shave Comments column should “wrap the text” so everything can be read on one screen.  I actually use this more as a shave journal.

I created this about four years ago, so it's showing some age:

Data input on the first picture is somewhat manual but the total uses and last use dates are automatic using arrays.  M$ Access would probably be a better program to use...
